Collates cutting edge evidence for the sick lobe hypothesis of breast cancer into a single, comprehensive volume. The theory of the sick lobe states that breast carcinoma is a lobar disease developing most often within a single lobe, meaning that, at an early stage, breast carcinoma occupies a limited, anatomically well-defined portion of the breast. This theory unites observed patterns from the genetic, developmental and morphological perspectives, into an overall concept. Breast cancer is a lobar disease in the sense that, at the earliest stages, the cancer is structurally confined to a single sick lobe. The subgross morphology of breast carcinoma is often complex, as multiple invasive foci are frequently present and the ductal system often contains an extensive in situ component.

Breast Cancer: A Lobar Disease, presents this hypothesis and its consequences. The body of evidence, pro and contra, generated in recent years is presented in this volume. A few quotations from the book Breast Cancer: A Lobar Disease by Tibor Tot will be used as an introduction to this chapter, which will be followed by the presentation of several cases of early breast cancers invisible on mammography and detected by ultrasound. The volume raises attention to the need of a completely new approach to breast cancer based on the knowledge collected on early breast cancer in the past two decades. The practical importance of such a book is underlined by the increasing number of breast cancer cases, and also the increasing proportion of early stage-cases.

By contrast, most breast cancer deaths occur in low and middle income countries (LMICs). The Breast Health Global Initiative (BHGI) developed resource-stratified guidelines as a framework for breast cancer management in limited resource environments. Phased implementation is a novel approach to guide strategic program development to save breast cancer lives.
